Two documents for shops that qualify welders: the performance qualification record you keep, and the certificate you hand the welder.

• Welder Performance Qualification (WPQ) record — laid out to the field set of ASME BPVC Section IX Form QW-484A. Welder name and ID, WPS followed, test coupon or production weld, and every essential variable in actual-value / range-qualified column pairs: process, backing, base metal and P-number, thickness, filler F-number, position, progression, transfer mode, polarity.
• Test results block — visual per QW-302.4, face, root and side bends per QW-462, alternative volumetric per QW-191, fillet fracture, macro examination — plus lab, supervisor and the certification statement.
• Presentation certificate — to issue to the welder, cross-referencing the WPQ record number.
• Worked example — a fully completed WPQ.

Usable for ASME Section IX, AWS D1.1 or API 1104 — the form records which code applied. A welder qualified to one code is not automatically qualified to another.
